Position Overview

Works a 7-on/7-off schedule (Mon 8:30am – Mon 8:30am. Must be available during regular hours). During scheduled on-duty weeks, the nurse is responsible for providing after-hours coverage and serves as the primary first-call nurse during all hours the office is closed. Furthermore, they are expected to provide patient visits, admissions, and support during regular business hours (M-F 8:30am – 5:00pm) on an as-needed basis. This role requires the ability to travel to patient homes and facilities as needed to provide care.

Position Responsibilities

  • Serve as the first point of contact for after-hours clinical needs, responding promptly and appropriately to patient and family concerns.
  • Promptly responds to all incoming calls, provides telephonic triage, and performs in-person visits as needed, including patient assessments, symptom management, admissions, and death visits.
  • Must be highly skilled in delivering quality nursing care, with the ability to assess, plan, implement, and evaluate patient needs in accordance with Heart of Georgia Hospice policies and accepted standards of practice.
  • Accurately observe, assess, document, and communicate patient conditions and care provided.
  • Complete patient admissions, re-admissions, and visits as needed.
  • Educate patients, families, and caregivers on care needs, disease progression, and symptom management.
  • Provide clinical guidance and supervision to other nursing and support staff.
  • Maintain knowledge and application of hospice standards, policies, and best practices.
  • Perform additional duties as assigned by the Clinical Manager.

Required Qualifications

  • Active, unrestricted Registered Nurse (RN) license in the state of Georgia.
  • Graduate of an accredited nursing program.
  • Minimum of 1–2 years of nursing experience required; hospice, home health, or acute care experience preferred.
  • Strong clinical assessment, critical thinking, and communication skills.
  • Ability to work independently and respond effectively in urgent or high-stress situations.
  • Current CPR certification
  • Valid driver’s license, reliable transportation, and ability to travel within the service area.

Physical Demands & Work Environment

  • Frequent standing, walking, bending, stooping, and reaching.
  • Ability to move, lift, and transfer patients of various weights with or without accommodation.
  • Ability to travel between patient locations, including driving at night and in varying weather conditions.
  • Prolonged periods of sitting, driving, and computer use for documentation.
  • Ability to communicate effectively (verbally and in writing) with patients, families, and healthcare team members.
  • Must remain alert, responsive, and capable of performing duties during overnight shifts.

Compensation and Benefits

$74, 880 – $76,960/ yr.
