At Heart of Georgia Hospice, we understand that true care encompasses more than just the patient. That's why we focus on addressing the holistic needs of our families, including their emotional and spiritual well-being. Our team of dedicated professionals is here to provide compassionate support, guiding patients and their families through every step of their journey. Care is available 24 hours per day, every day of the year.

Grief Support

Our bereavement team offers over 50 years of combined experience in counsel to individuals during their grief and healing processes that often occur during periods of loss.

Nursing Services

Patients are assigned a Registered Nurse to manage all aspects of a patient's care. The Registered Nurse Case Manager is a valuable resource for comfort strategies and works very closely with the entire interdisciplinary team to ensure advocacy for the patient's and family's wishes regarding the plan of care.

Medical Equipment

Durable medical equipment is a provided service to patients receiving hospice services. Equipment such as a hospital bed, bedside commode, and walker for example can offer comfort and safety when needed.

Medications

Medications related to the primary hospice diagnosis are included in provided hospice services. Comfort medications are very important during this time and our highly trained staff understand the appropriateness and timeliness of medication administration.

Social Workers

A Licensed Masters prepared Social Worker is available to help facilitate care for you or your loved one. Our Social Worker offers vast knowledge of resources to help you attain necessary assistance with end-of-life care such as personal caregiving services in home, facility placement, understanding advance directives, in addition to many other needs and/or services.

Supportive Education

All hospice clinical and supportive team members collaborate to provide the most comprehensive and effective strategies which are patient and family tailored and comprised of end of life and associated educational needs.

Chaplaincy Support

Our team of Chaplains and Licensed Bereavement Counselor offer compassionate comfort and assistance in a non-denominational capacity. End-of-life processes can be physically, emotionally, socially, mentally and spiritually challenging even to the strongest individuals.
